How to
Download

If you have Adobe Reader
installed on your computer, skip to #2.

Put your mouse cursor
over the name of the file. (see list
below).

Click.

Adobe Reader will open
the file.

You can then ask Reader
to save to your computer.

You can print out as
many copies as you like on your printer
and fill-in manually.

How to
Download

Place your mouse cursor
over the name of the file (see list
below).

Click.

A popup message box will
come up. Select SAVE and save to
your computer. Be sure to make a written
reminder of where you saved it on your
computer.

Do NOT select
OPEN. You will get asked for permissions
etc. You MUST use the SAVE function. If
you chose OPEN, simply cancel and start
over.

Once SAVED to your
computer, open it from your computer by
locating the file on your computer and
clicking it to open. You will need MS
Excel or compatible.
